The Goals of the Project
Strengthen County Diabetes Coalitions
This project will work with the existing diabetes coalition members in your county, aping members from schools and higher education; health professionals; businesses; political entities; agencies and non-profits; and recreation and faith-based organizations. The collaboration allows the project team to work with your county and its people to share ideas, methods, and activities, as your community works to build sustainable diabetes prevention programs.
Map County Assets and Develop Strategic Plan
The project team will map your county's assets and work with you to create a strategic plan to reduce problems linked with diabetes. The result will be a county-focused plan toward increasing healthy lifestyles for all residents while reducing the risk of diabetes.
Strategically Apress Diabetes Prevention
When everyone works together to apply a plan that increases knowledge, improves personal actions, enhances the environment and develops local policies to reduce health risks for all community people, the entire community benefits. The collaborative work of the project team, county coalition members, community stakeholders and local citizens contributes to a strategic plan developed from the community assessment, and therefore targeted to the communities' greatest needs.
Sustainable Actions to Reduce Diabetes
The community changes and policies that result from the plan's execution will result in sustainable actions to meet the needs of residents across their lifespans. The changes in local activities, environments, and policies will reduce diabetes and its risks for all residents and families at home, work, school, and play.
